Statute, standards and professional ethics require that all records and information regarding individuals, be kept in strictest confidence. Release of such records is done only with the informed consent of the individual or the individual’s guardian.
All employees of QUEBA HOME HEALTH SYSTEM (QHHS) are responsible for securing and respecting each individual’s records and information (verbal or written). Care must be taken to guard the same against unauthorized and unlawful use.
Records may be released only to employees of Queba Home Health System. Agencies or individuals other than QHHS must obtain written consent, prior to the release of said records or information.
Requests for information regarding individuals who live/or have lived at the Extended Family Services, shall be referred to the Central Records Office or the CEO/DOO.
Especially to be avoided is the discussion of individuals of Queba Home Health System (including but not limited to name) in the context of social gatherings.
Any violation of the confidence of individual’s records or information will be considered grounds for termination.
